How long does it take to start noticing a difference?

Avastin Lucentis Update 25 Comparative Studies Proliferate – France Joins the Fray


According to a story just published in APM Health Europe (September 14, 2009), a French study comparing Roche's Avastin (bevacizumab) and Novartis' Lucentis (ranibizumab) in the treatment of wet age-related macular degeneration (AMD) has enrolled its first patients.

The U.S. CATT Study (Comparison of Age-Related Macular Degeneration Treatment Trials) was launched in February 2008, and a U.K. study is also underway as well as similar studies being carried out in Austria, Germany and Spain.

In France, Professor Gisèle Soubrane of the Creteil hospital outside Paris suggested a comparative study as early as 2006 but failed to secure financing. Early in 2008, the Lyon group of hospitals (HCL) put forward a new protocol and applied for financing. There were various delays and setbacks but the GEFAL (Groupe d'Evaluation Français Avastin versus Lucentis) study is now underway.

The study is sponsored by an academic hospital in Lyon (Hospices Civils de Lyon), the government (via a national research founding program), and the national health insurance system (which reimbursed for Lucentis).

The first recruiting centre in Lyon enrolled an initial patient at the end of June. Contacted by APM last week, Professor Laurent Kodjikian of HCL said: "We have now enrolled 10 patients and by autumn we shall have at least 40 centres."

The GEFAL study is due to enroll 600 patients over a period of 12 to 18 months and they will be monitored for one year.

The French health products safety agency (Afssaps) has been notified that the study involves 46 centres (34 hospitals and 12 private centres).

The study will evaluate the safety and efficacy of Avastin. Provided Avastin is shown to be comparable to Lucentis, the findings should bring down the cost of AMD treatment. This is important since the disease affects elderly people, a population group set to increase.

According to IMS Health figures published in July, in 2008, Lucentis was the 15th most sold drug in France. Turnover was just below 150 million euros. Sales practically tripled between 2007 and 2008.

AFSSAPS RECOMMENDATIONS

Contacted by APM on the off-label use of Avastin since July 2006, Afssaps initially responded by saying it was not in a position to control such use and said it did not intend to communicate with doctors on the matter but later decided to look into the problem.

On Thursday, Afssaps published an information sheet on the off-label use of Avastin, drawing attention to the fact that in its current pharmaceutical form, the product is not adapted to intravitreal administration (insufficient stability data, etc.) and pointing out that the safety data on the use of Avastin in ophthalmology are limited.

The agency says adverse effects with the off-label use of Avastin have been "under-notified" but notes the risk of endophthalmitis/uveitis linked with the injection, and risk of tearing or detachment of the pigment layer of the cornea. Cases of thrombotic side-effects have also been reported.

The agency points out that other products have been authorised for the treatment of wet AMD. Until the results of the GEFAL study and others are available, Afssaps recommends caution in the use of intravitreal Avastin.

Sleep Disorders and their Homeopathic Treatment – Top 10 Homeopathic Remedies for Sleep Disorders

“SLEEP” is something one cannot live without. It is as natural as hunger. But if it is a natural response, why do so many people suffer from various sleep disorders? Majority of people at some time or the other during the lifespan suffer from some sort of sleep problem, which affects their general health.

It is concluded that for optimal body functions, good mood, healthy hormonal functions and overall health, sleep is very important. When some type of sleep problem occurs as a result of some external physical or mental barrier like high stress levels etc, once that barrier is expelled, the normal sleep pattern returns. When, however, the irregular / absurd sleeping pattern becomes almost a habit, one needs effective therapy to combat it. Before we go to how to combat sleep disorders, we need to know what different types of sleep problems we humans encounter. There are lot more types of sleep problems once we delve deep into the issue and not just the problem of insomnia or sleeplessness. Let’s see—

Types of Sleep Disorders—

1)   Oversleeping (Hypersomnia)- Medically, the normal sleep is said to be between 7 to 9 hours at night. If you’re sleeping far more than this, you do suffer from hypersomnia. In this problem, people suffer from excessive day-time sleepiness and drowsiness, and also extended sleep hours at night at times. Many people that suffer from this problem may have high anxiety levels, low energy levels or some memory troubles. In people with obstructive sleep apnoea too, in which the person suffers from sudden breathless spells during sleep, might suffer from oversleeping during the day. Also some over the counter medications, anti allergic drugs and alcohol can lead to oversleeping problem.

2)   Insomnia (Sleeplessness)– This is by far the commonest sleep problems and many people today are fighting the problem throughout the world. The list of causes is endless, starting from depression to wrong food habits. Primary insomnia is the one where there is no known direct cause behind insomnia. Secondary insomnia, the problem can be linked to some health disease like asthma, depression, heartburn, arthritis, etc.

The most important causes behind insomnia are –

n  Stress- that may be related to anything ranging from discord between the partners, death of a loved one, job problems etc.
n  Environmental factors like excessive noise, light or excessively hot ot cold temperatures etc.
n  Conventional drugs
n  Irregular sleeping pattern due to variety of causes.
n  Some kind of physical pain
The symptoms of insomnia include irritability, sleepiness during the day, memory or concentration problems, general fatigue etc.

3)   Snoring-Occasional snoring can occur with anyone and may not be significant. However, when it occurs in obese people and leads to some sort of obstruction to breathing, it is a serious issue and needs proper treatment. There are some side effects of snoring that has some kind of obstructive cause behind. They include frequent waking from sleep, poor night’s sleep and strain on the heart if the obstruction is severe.

4)   Night terrors and nightmares- Night terrors are common in children. Episodes of intense crying and fear during sleep with difficulty in arousing the child can almost diagnose the night terrors. There may or may not be any mental and/or emotional cause associated with such episodes in the children. But during homeopathic treatment, one has to consider all such factors very carefully.

5)   Parasomnias- These are sleep disorders that affect during arousals from REM (Rapid-Eye-Movement) sleep or partial arousals from NERM (Non-Rapid-Eye-Movement) sleep. Various sleep problems are encountered under this type, that are, sleep-walking, rhythmic movement disorders, sleep talking, nocturnal leg cramps, sleep paralysis, irregular heart rhythms, teeth grinding (bruxism), enuresis (bed-wetting) etc.  

6)   Sleep Related Eating Disorders- In this problem, the person shows abnormal eating pattern during the night. Night Eating Syndrome (NES) is characterised by eating during the night in full awareness and failing to fall asleep thereafter. People with hard and fast diet rules during the day are more vulnerable to such problem. Excessive work-related or relationship related stress is also linked to NES.

The above mentioned six types of sleep problems are by far the commonest. Recognizing that one has sleep problem and approaching a physician as early as possible can avert many adverse consequences. Also the conventional drug therapy has not quite succeeded in annihilating the sleep problems in their totality without making the patient addicted to the drugs. Also the drugs might act very temporarily and not give lasting relief, while leading to many side effects. Avastin Lucentis Update 7 BREAKING NEWS – NEI NIH Will Fund Comparative Study


Since posting this update on October11th, I have obtained further information about the upcoming clinical trial to be held to compare Lucentis with Avastin. For the details, see the Addendum below.

Avastin/Lucentis Update 7: BREAKING NEWS – NEI/NIH Will Fund Comparative Study

On October 6th, after the New England Journal of Medicine had gone to press, the National Eye Institute and the National Institute of Health announced that they will proceed with a comparative study of Avastin and Lucentis.

As noted in my previous three updates (see links below in the Author’s Note), I and others felt that common sense dictated that a comparative study be undertaken particularly because of the disparity in price between the two drugs – Lucentis priced at $1950 per dose, and Avastin (used off-label) at about a tenth of that price.

As stated on their web site: “The National Eye Institute (NEI) of the National Institutes of Health (NIH) will fund a new multicenter clinical trial to compare two drugs currently used to treat advanced age-related macular degeneration (AMD)... Lucentis (ranibizumab) and Avastin (bevacizumab).”

In addition, the announcement said: “In advanced AMD, abnormal new blood vessels grow beneath the retina. These abnormal blood vessels can leak blood and fluid, and can cause severe vision loss. NEI-supported research has helped establish that VEGF is an important part of the biological pathways involved in triggering and sustaining the growth of new blood vessels in the retina. Research also demonstrated that VEGF is present in higher levels in a number of eye diseases, such as AMD. NEI has supported more than 300 research studies at a cost of nearly $95 million to better understand the role of VEGF in eye diseases.”

At this point, two anti-VEGF therapies, Macugen (pegaptanib sodium) and Lucentis, have already been approved by FDA specifically for the treatment of advanced AMD. Paul A. Sieving, M.D., Ph.D., director of NEI, said, "We are pleased that NIH-supported research is stimulating the development and testing of new, effective treatments for AMD."

I contacted the NEI and learned that patient enrollment will begin after the first of the year.

As I learn more about this important clinical trial, I will continue to update my web site.

ADDENDUM

According to a report in the New York Times, if the drugs prove to be equivalent in treating age-related macular degeneration, it would probably mean that many eye doctors would switch to the cheaper drug, hurting Genentech's sales but saving the Medicare program hundreds of millions of dollars a year.

Both Avastin and Lucentis work in a similar way, and some eye doctors say they believe Avastin is equally as effective. However Avastin has not been tested in rigorous clinical trials as Lucentis has. So interest has been growing for a trial to compare the drugs. It is estimated that there are 200,000 new cases each year of the type of macular degeneration for which Lucentis is approved.

Dr. Maryann Redford, the administrator of the NEI grant for the study, said it will involve a two-year trial costing about $16 million. The trial will follow about 1,200 people divided into four groups: one group will get a Lucentis injection every four weeks, one group will get an Avastin injection every four weeks; and two groups will get either Lucentis or Avastin on an as-needed basis. Redford said the study will take four years to complete.

Dr. Redford added that cost is not the primary reason that NEI decided to fund the study. "Right now there is an uncontrolled experiment being done, in that many clinicians are using Avastin, but its safety and efficacy for this use hasn't been tested," adding, "If the fallout also is that it can be done much less expensively, great."

Hal Barron, Genentech's chief medical officer, said the company is not interested in paying for a study comparing Lucentis and Avastin because it is unlikely that Avastin is a superior treatment and because the company prefers to spend money developing new treatments. Barron added that 81% of patients pay less than $50 out of pocket for each Lucentis dose because they have Medicare or other insurance.

Eye floaters cure – surgical proceedure


If your eye floaters are extremely severe you might consider getting them removed by surgery. I wouldn’t recommend having them removed since there are a lot of natural and harmless ways to remove floaters in the eye, but specially when they become a possible cause of a retinal detatchment.

These are the most common ways to treat eye floaters by surgery:

Vitrectomy

This technique involves removing the vitreous humor of the eye, which is the liquid a human eye is filled with and where the eye floater are fleeting, to replace it with a saline solution.

As you can imagine this technique is extremely risky since some instruments have to be inserted inside of your eye.

YAG laser

That’s an obviously less aggressive alternative to the other one, but its risk is still quite high, achieving a success rate of 90%. This proceedure has fewer side effects as well and it’s performed all over the world.

That’s the most recommended surgical eye floaters cure by specialists, after the natural ones of course. The eye floater just gets vaporized by a laser and the smaller specks get deposited in the bottom of the eye.

All in all, it’s hard to tell wich one of them is the safest eye floaters treatment, but I don not recommend any of them unless it’s extremely necessary.
