To directly deal with a behavior that causes suffering was considered superficial, inadequate, but Bandura noticed how people who modified their behavior(ie. stopping drinking) in one way, had changes throughout other aspects of their lives. He published an article in 1963 on psychotherapy as a learning process, and it generated huge interest--including from Victor Meyer, featured in my last post. The response from more traditional psychotherapists was resolutely negative--BT was dangerous! It would lead to "symptom substitutions" and all that unconscious stuff stewing would burst out.

In meeting others with OCD in support groups or at the IOCDF conference, and reading the OCD-Support List, and comments from readers of this blog, I am taken aback by how, in mainstream treatment of OCD by therapists of a general psychodynamic bent, they still want to find the underlying cause of the obsessions and compulsions, to the exclusion of actually addressing the behavior itself. This can lead to OCD hell--especially for those of us with existential OCD questioning of everything we do anyway. And Cognitive Therapy can also fall into the same hell, with analyzing our thought distortions ad infinitum.

Male Body Image and Yoga

Nina recently asked me for my thoughts on body image from a male point of view. She said, “It seems to me for male yoga practitioners, it is not so much about getting a certain look as being able to perform showy poses. And, uh, you know what it is like to have a lot of limitations in that area. I thought that might be worthwhile/interesting to address.”

Well, Nina, I don’t agree with you that men are not going for a certain look or idealized physique. There are definite signals in our culture about what a healthy, strong man should look like, whether it is from the movie super heroes like Spiderman (he seems the closest to a yogi, to me), our professional athletes, or the hunky men pictured on the cover of magazines. And I personally felt this urge earlier in my experience with yoga, when I had little exposure to the philosophy of the practice and was enamored with the physicality of asana. I have always been on the thin side and was sometimes self-conscious about that, so in my early days I was particularly interested in the poses that built strength and muscle mass. So I’m guessing a lot of other men feel the way I did, whether they are interested in bulking up or reducing body fat.

But as I began to get serious about my yoga practice, it was helpful for me to observe my male teachers and notice that while some appeared to fit into the idealized view above, others clearly did not. And having the most ideal body did not necessarily equate with being the most grounded, happy, or successful person. For example, one of my male teachers was a large man, both in stature and in size, unlike the toned, lithe yet muscled ideal that I saw in Yoga Journal magazine. Yet this man was also very adept at asana and was, more importantly, kind, wise, and compassionate with his students—qualities I came to appreciate more and more over time.

So now when I notice that I am getting caught up in some internal dialog or dissatisfaction with how my body looks—like my arms looking too thin or weak and my belly looking flabby—I try to catch myself in the act and step back and take a broader view of my physicality and what my bigger goals are. Do I actually feel healthy in the body I have? Am I able to participate in the activities I love, like yoga, hiking, playing my violin, to the extent that is satisfying? Do I feel rested when I awake, and do I have adequate energy through the day to do my work and play well? I don’t expect to stop having opinions about how my body looks. But, from my yoga perspective, this is a reflection of the ego-personality part of my consciousness, called the ahamkara, which my yoga practice is assisting in quieting a bit over time!

As for the ability to perform showy poses, I usually refer to this aspect of a yoga practitioner’s experience as the need to feel “competent” in yoga. And I certainly used to feel this need myself, and it often led me to overdo it in class and end up with some mild injury. I am super tight in the hamstrings so I used to think how pathetic my forward bends were compared to those being done by my classmates. But over the years, I have softened my attitude towards needing to be really good at all the yoga poses, especially when the reality is that if I push myself too hard to achieve an external appearance of a full pose, I will most likely end up injuring myself. My forward bend practice is much more subtle now, with the feedback my body gives me determining how far I take things instead of my ego determining how I do the poses. And I have let go of certain “classic” poses like full Headstand and Shoulderstand after being in a car accident a few years back and needing to take better care of my neck. Sure, part of me misses how cool it felt physically and mentally to do these poses well. But another part of me does not at all, and I feel so much better these days when I listen deeply to my body!

And I certainly have spoken with many other men over the years who express distress at not feeling “competent” in the performance of yoga poses, sometimes even the beginning level poses! These men look around the classroom and see most of the women in the class appearing to easily come into Standing Forward Bend and Downward-Facing Dog, while they themselves struggle with limitations in movement and experience actual physical discomfort, at least at the start. It does not sit well with the other cultural expectations we men have, that they we should be smart, competent and successful (in a broad sense) in everything we do. And I’ve met guys who quickly gave up on yoga due to this dynamic for them!

I feel fortunate to have had teachers early on who acknowledged this possibility and redirected my mind to consider how the poses were feeling internally, shifting the focus from some exclusive achievement of an ideal outward appearance towards one of an inner dialogue and gradual changes appropriate for my body. Two of my teachers, Rodney Yee and Richard Rosen, both had strong practices in some ways, but were limited in other ways and could not do the “full” poses all the time. The cool thing about both of them was that they came up with creative and clever ways for me to try to approach the poses, without doing so in a careless, cavalier way that might lead to injury. For example, they had me use straps for some of the bound sitting poses like Marichyasana 3.

In addition, many of my teachers were masterful at sequencing practices that did a great job preparing my body to go as deeply, in a safe, mindful way, as I could toward those showy yoga poses. But the emphasis was always on the journey, not the destination. Sure, we were always heading somewhere and giving it our best effort. But I was learning to experience some reality other than the outward success or failure (do you hear the Gita, anyone?) of mastering the fancy, advanced yoga asana. I remember the first time Donald Moyer taught me seated forward bends using the bolster under the knees. I will never be able to wrap my hands around my feet in seated forward poses like Paschimottansana, but doing these modified versions finally allowed me to experience the “quieting” that I had heard about ad nauseam for years! And I was feeling that without holding onto my feet with my hands.

So, yes, men are influenced and motivated by body image, though perhaps not as overtly as women. And they are also strongly influenced by a need to feel “competent” or achieve “success” with challenging poses. But true competence in yoga does not mean the ability to do challenging poses. Instead, true competence means being able to practice in the way that is best for your particular body. In addition, moving toward less emphasis on the posture practice and introducing the other aspects that yoga has at its roots can make your yoga practice so much more appealing than just being “mystical P.E.,” as one senior yogi recently joked. I personally have enjoyed adding sound work, such as chanting on exhalations during my Sun Salutes, as well as developing better skills in subtler practices of pranayama and meditation.

People are always theorizing about why there are not more men in yoga classes these days—maybe body image plays a bigger role in this trend than we guys would like to admit! But, guys, no matter what size or shape you are, I want to encourage all of you to try to get over these issues and start to practice yoga to obtain all of the wonderful benefits of regular practice. I have had men of all sizes in my classes over the years, including guys who are carrying some extra weight. Some of them feel comfortable continuing in my regular classes, and I direct those who don’t to a specialty class a colleague started for Curvy Gals and Big Guys. So, no excuses, fellas!

Knee Replacements and Yoga

So I thought I would you give some background about the elective procedure total knee replacement and why you might elect to have it done. Professionally, I see a lot of total knee replacements in my work as a home health physical therapist, and I also have yoga students who come to my class either after the procedure or beforehand as they are preparing themselves for the surgery.

Although many people will never need surgery for arthritis of the knee, if you have severe joint damage, extreme pain that isn’t helped by other treatments, or very limited motion as a result of the condition, knee replacement surgery may be necessary. So if the arthritis pain in your knee worsens, and the exercise that once helped you feel better has become unbearable, your doctor may recommend a total knee replacement (TKR). Surgery for osteoarthritis can provide several benefits, including :
  • improved movement
  • pain relief
  • improved joint alignment
When you research the procedure, you will discover there are gender-specific knees, which are knee replacement models designed for women, and dozens of other options, too, including different materials, sizes and models from a variety of manufacturers. How do you choose? Generally, you don’t. Surgeons typically determine which implant they’ll use when they are in the operating room and actually looking at the structure and size of your bones.

Prior to surgery, however, you should have an informed conversation with the doctor about your options so you can ask good questions about why a particular model might be chosen, and determine if you’re comfortable with the doctor’s approach and experience or whether you’d like a second opinion.

When only a portion of your knee has severe arthritic wear and tear symptoms, as confirmed by diagnostic testing as well as subjective complaints, you may be recommended to undergo a “partial or unicompartmental knee replacement.” This may be recommended because it helps to straighten up the joint, which has changed its position as a result of osteoarthritis. Partial knee replacement can be more effective and durable if appropriate and are less invasive. Recovery time is less because there is less surgical trauma.When the entire knee joint is replaced that is called a total knee replacement, and the ends of the femur, top of the tibia and often the patella (knee cap) are fully replaced.

When you are ready to return to your yoga class, you should take time to talk with your teacher about your knee replacement. I will routinely ask these students a series of questions:

  1. How long ago was the surgery?
  2. Are you still in pain?
  3. Are you still in physical therapy?
  4. Do you have any hip or back pain (either before or after the knee replacement)?
  5. How much mobility do you currently have? Can you get up and down from the floor?
  6. Do you have arthritis in any other joints?

So be prepared to provide your teacher with this information. Knee range of motion will vary widely both in a recent post-operative knee as well as a knee replacement that is over one year old. A lot depends on how much motion you lost prior to the surgery and how hard you worked postoperatively. The answer to the question “Can you get up and down from the floor?” is important because it tells the teacher a lot about a student’s flexibility and strength. Knee flexion will vary considerably, but I have never seen a knee replacement with 155 degrees flexion nor have I ever seen someone able to do a deep squat. Whether this just happens to be my student demographic or not, I don’t know.

When resuming yoga practice, alignment is a big deal with knee replacements because post operatively you want to avoid torque forces through the joint because that affects cement in the joint. “Closed chain activities,” where your weight is shifted with your foot remaining on the floor,” where your foot is lifted off of the floor with subsequent weight bearing will affect the joint differently. For standing poses, I teach students with recent knee replacements to pick their feet up and then replace and position as opposed to pivoting to change directions. Down the road, you can introduce pivoting if it doesn’t cause pain or discomfort. Liberal usage of props; walls, chairs, blocks will assist the student in not over-doing too quickly.

Strengthening all of the muscles that cross the knee joint as well as secondary stabilizers is also important. This means front, back and side leg muscles. Attention to hip alignment and strength also translates to protection of the knee replacement. Kneeling is problematic but not necessarily injurious to the new knee. Finally, pay attention to the feet—where is the weight on the foot? A lot of individuals who suffer from arthritic hips and knees have feet that need some tender loving attention. Baxter in his prior posts talked about feet (see Your Feet on My Mind), and I agree that where the body meets the ground and how we stack up from there is crucial in protecting our joints for longevity, especially when we become bionic.

I would like to add that lot depends on your pre-surgical state of health. If you have been active up till the day of the surgery then your recovery time will take about three months till you feel like you have integrated the new knee into your body. This is a rough estimate, but it seems about right for a traditional total knee replacement. Minimally invasive and partial knee replacements have less trauma, so healing and function comes more quickly. For those individuals who have lost a lot of mobility and have developed severe range of motion loss and significant loss of muscle strength, the recovery time will be more arduous because of all the structural as well as cardiac changes.

Finally I want to emphasize this: talk to your surgeon before and after surgery to know what your particular limitations and precautions may be, and remember to share them with your yoga teacher. Remember this is an invasive surgical procedure and everyone heals in a different manner. Be kind to yourself and remember that with a lot of hard work you will regain function and improve the quality of your life. This the reason why you decided to undergo this surgical option in the first place.

Thoracic Outlet Syndrome (TOS) arises most commonly in people who have been in traumatic accidents, typically in car accidents, or who do repetitive jobs, such as computer work at a non-ergonomic workstation, so that is potentially a lot of people. What’s happening under the skin, so to speak, is that any of a number of structures could be getting compressed that should not be. More specifically, the cervical nerves that come out of your spine and come together to form the brachial plexus, (a kind of super highway interchange of different nerves from the cervical spine before heading down into the arm) can be pushed on by muscles and/or bones and lead to the symptoms described above. The same fate can befall the subclavian artery, the large blood vessel from the heart that feeds the arms, as it moves through the area of the side neck, under the collar bone, and out into the upper arm. 

A commonly cited aggravator of this condition is poor posture, which we have mentioned in many other contexts as contributing to other body pain conditions, and for which yoga is such a great antidote. In fact, most people diagnosed with Thoracic Outlet Syndrome will first be referred to physical therapy for postural re-alignment and stretches to open up the area of the neck and upper chest in order to create more space around the brachial plexus and arteries and veins to lessen or eliminate the symptoms associated with the condition. One muscle group that is often implicated in compressing the nerves and blood vessels is the scalenes, specifically the anterior and medial bodies of this three-muscle trio.

It would be worth your while to review an anatomy book if you are interested in understanding these muscles, which movements they normally create, and how one might stretch them to relieve tension on our delicate nerves and arteries.

Yoga could be a great adjunct to this healing process, since many of our sequences and poses address this area of the body nicely. Even simple warm up exercises like shoulder rolls, where you bring your shoulders forward, then up, then back and down, can start to improve this area immensely. I start class with this kind of attention to the upper chest quite often. Basic attention to Mountain pose and supported reclining backbends would be good starting places as well. And there are a few postures that directly affect the scalenes, such as the head positions in Bharadvajrasana seated twist in which you rotate and then side-bend the neck.

Conversely, since folks with Thoracic Outlet Syndrome often get symptoms when their arms are held overhead, some caution must be used when doing yoga poses with the arms ups, like Warrior I pose, for instance, especially if you intend to hold the pose for a while. I would not be surprised that a few people out there may have become aware of their positional symptoms after starting yoga due to the demands of the poses on the arms. (Yoga would not “cause” Thoracic Outlet Syndrome, but could alert you to its presence in such positions as Mountain pose with arms overhead, Urdhva Hastasana.) If such postures do bring on pain or numbness, it might be more prudent to experiment with dynamic versions of such poses, moving the arms up and down with the breath to see if the dynamic action precipitates symptoms. If not, you could continue to practice this way or even add in short holds as long as no symptoms arise. 

By moving and liberating held tension in this area, ultimately you may be able to relieve the compression that is underlying the Thoracic Outlet Syndrome. In fact, most patients with this diagnosis find some improvement with modalities like physical therapy and, I suspect, yoga as well, and very few have to resort to surgical intervention to find relief.
