Avoidance is not Benign in OCD or for the Body

I did an exposure by calling a physical therapist about my shoulder pain.  Part of my health anxiety assuming I must know what is wrong with me, choose the right kind of therapy, and "know before knowing."  The PT said I have a frozen shoulder, which in part arises from avoiding moving a shoulder that is painful, and the more you avoid, the more stuck it gets.

What a metaphor for mindfulness, facing fears, doing exposure therapy for OCD!

Avoidance can bring on the very thing we fear:  anxiety, pain, suffering.

Of course, frozen shoulder is somewhat of a mystery, as to how it arises in the first place. Uncertainty stirs my OCD.

But I am proud of myself for choosing to do something, one step at a time, rather than continuing to wait until the "perfect " time to call and the perfect kind of professional(family doc, physiatrist, physical therapist, massage therapist, chiropractor).

Yoga for Foot Pain

I am a 67-year-old female in good physical condition. About 10 years ago, I slipped down a steep hill and came to a stop half way down the hill, using my foot to stop. I immediately had severe pain in the arch of my foot to which I applied ice. Every since then, I have had pain in the arch of my foot, to varying degrees. I now have that pain wrapping around from the arch to the top of my foot and pain that wraps around my ankle. Even the bone on the medial side of my foot, above the arch, becomes sore at times. I frequently wake up in the middle of the night with arch pain. When I walk fast at all, I get shooting pain in my arch. I have been to the orthopedic doctor and they have injected steroids into the tendon sheath in the bottom of my foot and also by the ankle. These injections have helped for a short time. I don’t want any more steroids. Are there any yoga exercises that I could be doing to strengthen this area and to decrease the inflammation that is there? Any suggestions would be very much appreciated.

I’d like to start by acknowledging how challenging traumatic injuries can be to our bodies, and how they sometimes don’t easily heal and allow us to feel like our old selves again, at least not on the timetable we would prefer! Our reader’s situation appears complicated in the sense that the pain she experiences moves around, is aggravated by certain activities, has been around a long time, and even wakes her up at night. And despite seeing an orthopedic specialist and getting some steroid injections to decrease inflammation in the tissues of her foot, her pain persists. It turns out the feet are quite complex parts of the body. There are 52 bones in the feet, and each foot has approximately 33 joints, each of which can move in multiple directions. A good place to start would be with any “warm-up” practices that involve lying on your back or belly and moving your limbs and feet around. An example would be Legs up the Wall pose (Viparita Karani), in which you could add in movements of your feet and ankles to promote mobility of the joints and soft tissues of your feet. Other examples of poses where there is no or very little pressure on your feet include Thread the Needle pose, Locust pose (Salabasana), and Reclining Twist pose (Jathara Parivartanasana). In all these poses, you can deliberately choose how you want to position your feet and ankles—whether you want to spread or curl your toes or vary other foot movements and positions—to bring greater awareness to how your feet are doing moment to moment. Once you are comfortable in these poses and your foot pain doesn’t flare, you are ready to move on.

There are several basic yoga poses that begin to require more of your feet in terms of dealing with forces to the soles, the tops, and the edges of your feet. As you introduce this new group of poses, you will again be monitoring how your feet respond during the practice, as well as after the practice, such as overnight. Some of my initial choices include:
  1. Reclined Leg Stretch pose (Supta Padangustasana)
  2. Bridge Pose (Setubanda Sarvangasana)
  3. Seated Crossed-Legs Pose (Sukasana)
  4. Cobblers Pose (Baddha Konasana)
With Reclined Leg Stretch, you are still not bearing weight but you are bringing pressure onto the sole of your foot with the strap. You can vary the location of the strap; I like the heel pad, the mid-foot, or the ball of the foot. And you can vary the intensity of the pull of the strap to gauge how much your foot can handle.

With Bridge Pose, you are beginning to bear weight on your feet, but not your entire body weight, so this pose provides is good way to test the waters to see what your feet are ready to handle. The other seated poses put a bit of pressure on different parts of your feet and can help reveal areas of ongoing tenderness that might need a slower approach towards full function.

Full Weight-Bearing Poses

When you are feeling ready to stand up and see how your feet handle more challenging situations, you can begin to explore the basic two-legged standing poses. Mountain pose and all of its variations—arms overhead, side bending, and so on—are the starting place and a good home base to return to for this group of poses.

From there, move on to the classic two-legged standing poses, such as Triangle pose (Trikonasana), Warrior 1 and 2 (Virabrdrasana 1 and 2), Extended Side Angle pose (Utthita Parsvakonasana), Pyramid pose (Parsvottansanas), and Widespread Standing Forward Bend (Prasarita Padottanasana). Each of these poses will bring slightly different challenges and forces to your feet and ankles. You might add them gradually over time and assess how each one affects the feet.

Once poses on two feet are becoming easier, add more challenge by working on one-legged poses, such as Tree pose (Vrksasana), Half Moon pose (Arda Chandrasana), Eagle pose (Garudasana), Warrior 3 (Virabradrasana 3), and Dancer pose (Natarajasana). Be willing to go more slowly with these pose, however, as they will focus more force and pressure onto your feet and ankles, and may require more time and attention to get better at.

Dynamic Movements

In real life, we are moving around on our feet quite a bit, walking, occasionally squatting, and periodically dancing, cycling, or running. You can ready and train your feet for these variable movements by practicing dynamic yoga sequences. The most obvious choices are Sun Salutations and Moon Salutations, but I suggest that you can also allow yourself some creative license to invent your own dynamic movement using yoga poses. Keep a playful, yet focused attention as you move in and out of various yoga poses with clear awareness of your feet.

As requested, I am also checking with our program experts to see if we have any flexibility regarding the timing of this coding change (i.e., whether it can be applied retroactively or whether it can go into effect before January 1). If you have any questions, please let me know. “


From page 615 of the Federal Register, October 30, 2009:

We note that HCPCS code Q2024 (Injection, bevacizumab, 0.25 mg) was implemented effective October 2009 and represents a different dosage descriptor for the same drug described by HCPCS code J9035 (Injection, bevacizumab, 10 mg). Further, HCPCS code Q2024 has been replaced with HCPCS code C9257 (Injection, bevacizumab, 0.25 mg) effective January 1, 2010. In accordance with our CY 2010 policy to make a single packaging determination for a single drug, we are applying the methodology described above to bevacizumab and are assigning the applicable bevacizumab HCPCS codes the same packaging status for CY 2010. HCPCS codes C9257 and J9035 are included in Table 35 below.


In what I read from this in plain English, the reimbursement for the 0.25 mg dosage of Avastin, used in treating AMD, will revert on January 1st to the reimbursement in place before the change over on October 1st, or about $50.00, which I believe is what it was before the change.

Further, both CMS and Senator Kohl’s office, along with several ophthalmic organizations (AAO, ASRS, among others), continue to look into/request that the change in policy be implemented before January 1st.

  6. You must either be in the photo or have taken the photo (that way, I can have your official permission to use it).

This is a rolling contest  and there is no deadline. I'll keep taking and using photos as long as I'm blogging. So take your time and submit a quality photograph rather than just sending in whatever snaps you have on your phone. I'm going to use the same standards for photos of men that I use for photos of women. Typical reasons a photo is rejected include:

  • Photo is too small (resolution is too low), so it will be blurry on the blog.
  • Photo is out of focus, partly out of focus, and/or lighting is poor, so person is in shadow or whole photo is too dark.
  • Figure is too small within the background, (you will look like an ant on the blog, even though, yes, that mountain is really pretty).
  • A body part or parts is awkwardly cropped off (where are your feet and/or fingertips?), or your face isn't showing in a pose when it normally would (the pose was taken from an odd angle or you are wearing a hat and sunglasses).
  • The facial expression seems unhappy, uncomfortable, or pained.
  • The environment is messy (props, plastic water bottles, bits and pieces of other students in the classroom).
  • Alignment of the pose looks unhealthy in my opinion, for example, the head and neck are not in line with the spine.
  • The photo doesn't feel inspiring or uplifting to me. (Hey, if we want to get positive images of older people doing yoga out there, the images need to feel positive.)

Although I don't by any means require professional photos, typically a quick snapshot done with your phone in a classroom or out on a hike—unless it is really special—isn't going to make the grade. Trust me, I've seen a lot of them at this point. So if you want your images to be seriously considered, take some time in creating them and take multiple shots of a single pose to get a good one. Arrange the setting and move that plastic water bottle somewhere else. Have someone look at your alignment and give you some cues. Take off your hat and sunglasses and smile. Or, at least relax your face.

He lit up with a smile and then went off in much better spirits to write a list of resolutions for his teacher (and keeping the “pet more dogs” resolution throughout the year did turn out to be a lot of fun.) I’m bringing this up now, because if you are planning on making any New Year’s resolutions regarding yoga this year, I’d advise you to take the same lighthearted approach.

If you want to start a home practice, rather than deciding to do full-length class everyday—a rather overwhelming commitment—think small. As my son did, try to come up with a resolution that will be easy to keep and fun to do. How about:
  1. Do one Downward-Facing Dog pose a day five days in a row for one week. (You can pet yourself afterward.) 
  2. Look through a yoga book and find a picture of a pose you’ve never done and just try it. (Be sure to laugh if you get totally confused or fall out of the pose.) 
  3. Download a yoga nidra practice or guided relaxation onto your iPod and try it once. (You might become addicted.) 
  4. Clear some wall space, figure out what to use for props, and set yourself up for Legs Up the Wall pose at home. (If you decide to do again some day, you’ll be ready.) 
  5. Practice seated meditation for five minutes a few times in a week. (If it feels good, try it for a second week, then a third, then....)

That’s what happened to me: I started practicing just a few poses at home, and gradually over time my practice grew organically. But even if your practice stays small, as The Bhagavad Gita says: “No step is lost on this path, and no dangers are found. And even a little progress is freedom from fear.”
